I wish to apply as a Teacher Trainee and I agree to assume full responsibility for any injuries to me that may occur during the training. I am in good health and any physical illnesses or ailments are clearly stated above. Being accepted into this course does not guarantee that I will be certified to teach yoga or that I will teach at the Integral Yoga Institute. I understand that I am applying for acceptance into this program and will not be confirmed until I receive an email acceptance letter from Integral Yoga, SF.

I understand that if I cancel before the start of the program, I forfeit up to $125 of the deposit and the remaining balance will be refunded. Once the program begins, the deposit and program tuition is non-refundable and non-transferable. Payment and registration for the training can be made on our website using MindBody Online.
